Health and Wellness Waiver Explanation
The H&W Waiver is designed to provide an alternative to NF admission for Medicaid-eligible persons ages 59 and younger with a disability by providing supports to complement and supplement informal supports for persons who would require care in an NF if waiver services or other supports were not available. The services available through this waiver are designed to help members remain in their own
homes, as well as to help individuals residing in NFs to return to community settings, such as their own
homes, apartments, assisted living or adult family care.

Soon to Offer- Indiana Pathways to Aging Waiver
We will soon also provide case management services for the Indiana Pathways to Aging Waiver for those counts 60 and over.
Indiana’s PathWays for Aging Program is a new, managed long-term services and supports initiative for Medicaid enrollees (or those dually eligible for Medicare and Medicaid) who are 60 years or older and meet age, blindness or disability criteria.
FSSA is partnering with experienced health plans to manage care coordination with an emphasis on ensuring that older Hoosiers will have access to quality care and can achieve similar health outcomes regardless of race, ethnicity, geography, etc..
